c) Broker Compensation. For insurance products placed via surplus lines carriers or non-admitted markets, broker compensation fees may apply and will not exceed US $200 per policy term.


d) Late Payment. Late fees may apply on insurance policies as determined by the insurance carrier. Non-payment when due may result in policy cancellation and possible reinstatement fees. Late fees cannot be reversed or waived.


e) Financial Privacy. We comply with the Gramm‑Leach‑Bliley Act (GLBA) for applicable consumer financial services, including providing initial and annual privacy notices and opportunity to opt-out of certain information sharing.


f) Insurance Consumer Reporting. You acknowledge that insurance carriers may obtain reports on driving history or claims for underwriting via third-party sources (e.g., LexisNexis). You may request a copy of your report by contacting the provider.


g) HIPAA Compliance. As required by law, we comply with the Health Insurance Portability and Accountability Act of 1996 and any applicable state regulations regarding protected health information. We will not release your health-related information without your written authorization unless required by law.


h) Non-Discrimination. We comply with applicable federal and state civil rights laws and do not discriminate on the basis of race, color, national origin, age, disability or sex in providing insurance services.

Credit card Processing

​Some of our insurance products require payment made to the insurance carrier directly from Insurance Group of Nevada and not the insured. If you request payment to be made by credit card to pay for your insurance premiums then Insurance Group of Nevada will send you an electronic invoice that will include a 4% processing fee. The processing fee is necessary to ensure the full payment amount is made to the insurance carrier.



Broker Compensation

​Insurance coverage provided through the use of non-admitted insurance, surplus lines carriers are subject to fully earned broker compensation fee. The fee amount is payable to Insurance Group of Nevada. The fee amount does not exceed $200 per policy term. 



Late Payment

​Insurance policies that are sold through Insurance Group of Nevada may be subjected to a late fee as determined by the insurance carrier. The late fee payment is required to be paid to the insurance carrier. If payment is not made when due, the insurance policy is subject to cancellation. If the policy is cancelled there may be an additional reinstatement fee charged by the insurance carrier. Late payment fees can not be reversed or waived. 



Gramm Leach Bliley Act

​Gramm Leach Bliley Act is also known as the Financial Services Modernization Act of 1999. Subtitle A: Disclosure of Nonpublic Personal Information, codified at 15 U.S.C. §§ 6801–6809) The Financial Privacy Rule requires financial institutions to provide each consumer with a privacy notice at the time the consumer relationship is established and annually thereafter. The privacy notice must explain the information collected about the consumer, where that information is shared, how that in- formation is used, and how that information is protected. The notice must also identify the consumer’s right to opt out of the information being shared with unaffiliated parties pursuant to the provisions of the Fair Credit Reporting Act. Should the privacy policy change at any point in time, the consumer must be notified again for acceptance. Each time the privacy notice is reestablished, the consumer has the right to opt out again. The unaffiliated parties receiving the nonpublic information are held to the acceptance terms of the consumer under the original relationship agreement. In summary, the financial privacy rule provides for a privacy policy agreement between the company and the consumer pertaining to the protection of the consumer’s personal nonpublic information. On November 17, 2009, eight federal regulatory agencies released the final version of a model privacy notice form to make it easier for consumers to understand how financial institutions collect and share information about consumers.



Insurance Consumer Reporting 

​Many insurance carriers utilize Lexis Nexis to share and transmit driving history, and insurance claims. These items are used in underwriting the cost for insurance products. To request a copy of your report call Lexis Nexis at 1-800-456-6432. Insurance Group of Nevada utilizes a secure and proprietary insurance verification system. The report is obtained during the quoting process and does not have an impact on consumer credit reports. The information obtained is used to verify and confirm consumers identity, and property information. 



Fair Credit Reporting Act (FCRA) 

Fair Credit Reporting Act Notice:

In making an application for insurance it is understood that an investigative consumer report may be prepared whereby pertinent information concerning your character, reputation, personal characteristics and mode of living may be obtained. Information as to the nature and scope of this report may be obtained upon written request.


Any request for personal or business insurance quote, Insurance Group of Nevada utilizes both public and private data sources to confirm the validity of any information that is given to Insurance Group of Nevada. 



​HIPAA Disclosure 

Insurance Group of Nevada is a licensed insurance entity with the State of Nevada Department of Insurance (DOI). 

HIPAA means the Health Insurance Portability and Accountability Act of 1996, codified in 42 USC §§ 1320 - 1320d-8 and 45 CFR Parts 160 and 164. As required by State and Federal law any information submitted, held, managed by Insurance Group of Nevada will only be released with your expressed and written notification, unless Insurance Group of Nevada is ordered to comply by court order. Health information is obtained by Insurance Group of Nevada at the request and demand of an insurance carrier that we are authorized to represent. Our policy is to never share confidential information about our clients with any person or entity unless required by our insurance carriers for underwriting and claims purposes or court order. 



Fraud Statement 

​​Any person who knowingly presents a false or fraudulent claim for payment of a loss or benefit or knowingly presents false information in an application for insurance is guilty of a crime and may be subject to fines and confinement in prison.


Final Insurance Premium Rate

Personal Insurance rates are subject to final underwriting review and approval. Negative credit standing and in adequate prior insurance is subjected to a higher rate. The price quoted may change until the company obtains final approval from the insurance carrier's underwriter. Quoted rates are only an estimate. Additional factors like credit, prior insurance history, location, marital status is all factors in the insurance premium.

Insurance Group of Nevada meets or exceeds the applicable privacy and security requirements of: (1) the Agreements; (2) the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and its implementing regulations at 45 C.F.R. Parts 160-164, (3) applicable state law governing data protection; and (4) the Payment Card Industry Data Security Standards (where applicable.)



Important Information about Procedures for Opening a New Account

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.

What this means for you: When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.
